Thank you for your interest in working with UMass Dining! The purpose of this form is to gather information regarding your farm or business.
Please fill out this form and s
ubmit a PDF or Word document to kwicks@umass.edu containing a comprehensive list of products. For each product, please indicate: seasonal or year-round availability, capacity (e.g. lbs/week), and price.
Once we have received all of the information, we will review it and be in touch as soon as we can.
